Add a workqueue so that I/O can be queued up to be flushed from a separate thread when necessary. This is necessary because we will want to flush I/O from end_io() which can be called in irq context, and flushing I/O can cause the caller to sleep. A new per-snapshot spinlock pe_lock is introduced to protect queued_bios.
